Program application focuses on problem identification, development and
implementation of a corrective action plan, evaluation for desired outcomes and
redirection if the result does not achieve established goal. Current Program activities
which will involve ambulatory care practitioners include, but are not limited to:
1. Review of medical records
2. Review of office site
3. Review of member quality complaints
4. Review of member PCP satisfaction
5. Review of adverse events
6. Review of practice accessibility and availability
7. Participation in Medical Care Management, Utilization Management,
Preventive Health, Health Promotion and Health Improvement Program
activities
8. Participation in focused Quality Improvement initiatives as appropriate based on
membership panel demographics and epidemiological characteristics.
Examples of ongoing programs include:
a) Diabetes -retinal exams, quarterly HgbAIc testing and management,
cholesterol screening and management, microalbuminuria screening and
foot examinations
b) Asthma- controller medications
c) Cardiovascular Disease- includes the spectrum of hypertension, CHF and
post-cardiac event management
d) Prenatal - high risk assessment, prenatal/postnatal interface, C-Section
and VBAC education
e) Breast Cancer Screening - mammography screening services
f) Chlamydia Screening- of sexually active females
g) Child and Adult Immunization - immunizations appropriate to the age
range of the practitioner’s member population
h) Council for Affordable Quality Healthcare- Saving Antibiotic Strength and
heartBBeat for Life
